New: Oxford Out-of-hours GP record sharing initiative

The Leys Health Centre is part of an organisation of Oxford city called OxFed (Oxford Federation for General Practice and Primary Care).

OxFed was set up in 2015 by local GPs to help improve the health and care of people of all ages and backgrounds who live, study or work in Oxford.

OxFed has launched a new Out-of-Hours GP record sharing initiative. This will help patients receive safer, better coordinated care when their usual GP surgery is closed. The Leys Health Centre is pleased to be part of this initiative.

To find out how it will affect you please, or to find out more about how to opt-in or opt-out of this initiative, please see the Patient Information Leaflet.
